I hereby certify that all the entries have been made correctly. I am medically fit to undergo the course. In case of any accident or injury I shall not hold the Institute or any of its staff wholly or partially responsible for it and shall not make any claim to that effect. In case of loss of equipment, I shall pay for the same as per institute`s rule.

I, Parent of the student who's credentials and photo is pasted above, agree and understand that snow skiing and snowboarding are adventurous activities which may result in injury to my ward and I hereby release all persons or entities connected with PCHSS / or its staff from all liability for any injuries, death, or damages and from any claim by me, my family, estates, heirs and assigns arising in any way from my wards’ participation, and contractually promise not to sue PCHSS / or its staff for any injuries, death or damages.

In addition, I agree to indemnify, defend, and hold PCHSS / or its staff, its representatives, and employees of and from any claim, action, harm, injury, damage or loss to persons and/or property which may be made by my ward or of that the undersigned ward may cause or contribute cause to any third party.

I hereby certify that all the entries have been made correctly. My ward is medically fit and can undergo the skiing and any other adventure activities at my risk and responsibility.

In case of loss of equipment or breakage of skiing equipment by my ward, I shall pay for the same as per the institute`s rule.

I am aware that this contract is legally binding and that I am realising legal implications by signing it.

I understand and agree that the skiing courses are planned based on the patterns of snowfall. In case of insufficient snow, an alternative activity trekking will be conducted, but fee will not be refunded.
